Job Description
Health Home Care Managers
Key Responsibilities
Health Home Care Managers are responsible for assisting individuals with developmental disabilities and complex medical conditions by organizing the high-quality daily care services they require. The Care Manager helps the individuals’ families understand their chronic condition(s), manage their symptoms and ensure that all of their treatment team providers involved are working together. After an intake assessment, care managers will take the reins in developing a long-term care plan that suits the individual’s goals. On a typical day, our care managers:
* Meet with individuals
* Connect with families
* Obtain available community resources for children, adults and families for public assistance, housing, child care, food pantries, transportation, and much more
* Submit insurance paperwork
* Monitor individual progress
* Provide supportive, recovery- and wellness-directed coaching and crisis counseling in-person and via telephone communication
* Communicate with doctors and other health professionals, educators, and child welfare services that are involved in the individuals care
Care managers usually balance a caseload of multiple individuals such as developmentally disabled individuals. Care managers should have organizational skills including computer skills, basic knowledge of the children and adults’ public health/mental health system, child welfare, public entitlement and social service programs. Care Managers must be reliable self-starters with the ability to work both independently and part of an interdisciplinary team. A Health Home Care Manager must be flexible in availability to work evenings, and after school to meet employed caregivers’ availability.
