Job Description<br \/><\/strong>Title: Health Home Care Managers<br \/>Key Responsibilities<br \/>Health Home Care Managers are responsible for assisting individuals with developmental disabilities and complex medical conditions by organizing the high-quality daily care services they require. The Care Manager helps the individuals\u2019 family understand their chronic condition(s), manage their symptoms, and ensure that all their treatment team providers involved are working together. After an intake assessment, care managers will take the reins in developing a care plan that suits the individual\u2019s goals. On a typical day, our care managers:<br \/><br \/>\u2022 Meet with individual<br \/>\u2022 Connect with families<br \/>\u2022 Obtain available community resources for children, adults, and families   for public assistance, housing, child care, food pantries, transportation, and much more<br \/>\u2022 Submit insurance paperwork<br \/>\u2022 Monitor individual progress<br \/>\u2022 Provide supportive, recovery- and wellness-directed coaching, and crisis counseling in-person and via telecommunication<br \/>\u2022 Communicate with doctors and other health professionals, educators, and child welfare services that are involved in the individuals care<br \/><br \/><br \/>Care managers usually balance a caseload of multiple individuals who are developmental. Medically Fragile, Serious Emotionally Disturbed, experienced Complex Trauma, and have HIV\/AIDS. Care managers should have organizational skills, computer skills, basic knowledge of the children and adults&#8217; public health\/mental health system, child welfare, public entitlement, and social service programs. Care Managers must be reliable self-starters with the ability to work both independently and part of an interdisciplinary team. A Health Home Care Manager must be flexible in availability to work evenings, and after school to meet employed caregivers\u2019 availability.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><br \/><\/p>\n","protected":false},"excerpt":{"rendered":"<p>a.
