Little Treasures-Petits Tr\u00e9sors SLP, PLLC (Little Treasures)\nis a community-based agency with the ability to provide comprehensive treatment\nthrough caring providers. We provide exceptional services to children and\nadults with special healthcare needs, by connecting thousands of developmentally\ndisabled individuals who have a wide spectrum of health conditions to the\nservices needed to remain in their community. We have dedicated ourselves to providing services to children\nand adults with developmental disabilities, complex traumas, HIV\/AIDS, and\nmedically fragile. Our goal is to ensure that reliable care managers deliver\nservices that will help individuals regain independence, improve their quality\nof life, and promote optimal care.<\/p>\n\n\n\n
Little Treasures Health Home Care Managers work closely with\nindividuals who require specialized medical, educational, and rehabilitative\ncare to help them and their families better\nunderstand and manage their needs. Once individuals are enrolled in a Health Home Care\nManagement Program, a specially-trained Care Manager will be assigned to work\nwith them to develop a care plan that will meet all their physical, mental\nhealth, and social service needs. \nA care plan identifies the services the individual needs, to put him\/her\non the road to better health. The\nHealth Home Care Manager will help individuals find the services and programs\nthat are appropriate for them. <\/p>\n\n\n\n
What is a Health Home?<\/strong><\/p>\n\n\n\n A\nHealth Home is not a place. It is an incredible team of health care\nmanagers and service providers working together to ensure individuals living\nwith chronic conditions receive the best care and services needed to stay\nhealthy.<\/p>\n\n\n\n A\nHealth Home is a NYS Medicaid funded program and\nis available to Medicaid-eligible children and adults who have chronic medical\nand mental health conditions. \nThese individuals may benefit from the careful planning and case\nmanagement provided by Health Home Care Managers. New York has two approaches to enrolling eligible\npatients in Health Homes. Health Home enrollment can be passive, where MCOs will\nautomatically assign enrollees to Health Homes, or active, where potentially\neligible individuals sign themselves up. Little Treasures\u2019 Care Management Agency was created to make\nsure its enrollees get the best care and services they need. Our Health Home\nCare Managers will help individuals and families complete the necessary\npaperwork and will explain everything they need to know in detail. <\/strong><\/p>\n\n\n\n If each enrollment options present any\nchallenges that need to be addressed as enrollment procedures are developed.\n , our Care Managers will support whichever method that ensures\nall eligible individuals are aware of the Health Home Program, while offering\nthe flexibility for individuals to choose the care that best meets their\nneeds. <\/p>\n\n\n\n Enrollment in the Health Home Program\nis voluntary. You do not have to\nenroll in a Health Home Program, if you do not want to. Once you have decided to enroll, you\nmay choose to dis-enroll at any time. <\/p>\n\n\n\n How can a Health Home help me?<\/strong><\/p>\n\n\n\n Once\nyou are enrolled in a Health Home, your Care Manager can:<\/strong><\/p>\n\n\n\n What if I am part of\na Waiver or Early Intervention Program? <\/strong><\/p>\n\n\n\n If\nyou are part of a waiver or early intervention program and are interested in a\nHealth Home, you will simply get transitioned to a Health Home and sign all\nnecessary consents.
